LEADERSHIP: COUNCIL AND CHAPTER DEVELOPMENT

How frequently and effectively does the Chapter Council meet?

A Chapter leader (i.e. Chapter President, President-elect, designee of the Chapter President and/or Chapter Executive) has represented the Chapter at all three meetings of the BOG in 2009.
 

In 2009, the Chapter Council has developed or reviewed a Strategic Plan with clear, measurable goals.  Please forward plan via email to Chapter Affairs (chapters@acc.org).
 

Chapter has submitted a status of the goals set for 2009 to the BOG Steering Committee through Chapter Affairs.  


Briefly elaborate on what 2009 Chapter goals were accomplished, and your plan to keep goals relevant to your membership. 


How frequently and effectively do the Chapter President and Chapter Executive communicate.

 

Name the vehicles and frequency of communication to your chapter members.

 

How do you collect feedback on your communications, and how do you use that feedback? 

 

The Chapter has an emergency plan addressing communications in place. 


The following section is MANDATORY to be a Chapter Awardee:

Chapter President and Chapter Executive use the Chapter Operations Manual.

The Chapter is in compliance with all of the good business practices outlined in the Operations Checklist within the Operations Manual.   

IRS classified tax exempt status is current and in good standing.  

Articles of incorporation are current.  
Employer Identification Number (EIN) is on file with Chapter Affairs.
Annual Corporate status is on file and in good standing.
Chapter bylaws have been reviewed and are current and in compliance with applicable state and ACC National.  
Chapter bylaws are posted on its website. 
Financials have been examined by the Chapter President, Chapter Treasurer, and Chapter Executive and reviewed with the Chapter Council on a quarterly basis. 
Chapter Executive is operating under an ACC approved contract. 
Chapter Executive has been reviewed by the Chapter President within the last twelve months and his or her goals were defined for next twelve months. 
Chapter has clear, transparent financial reports and a defined reporting system for Chapter leaders in place. 
Chapter Council has approved an operating budget for the next fiscal year. 
Chapter has filed copies with Chapter Affairs of their most recent end of year financial statement and next year budget, along with their 990, Articles of Incorporation, Chapter Bylaws and management contract. 
An independent accounting firm has audited or prepared the Chapter financials and/or tax returns. 
Chapter carries Directors& Officers insurance to limit the personal liability of their leaders. 
 

ACTIONS: MANAGEMENT & MISSION 

 

Membership:
Chapter has welcomed FITs into their membership (i.e. position on Council, special meetings, targeted section of a meeting, special invitations to general meeting, the bylaws, and other inclusive manners).   Please explain how you have done this and how it has worked for your Chapter.

Chapter has welcomed CCAs into their membership (i.e. position on Council, special meetings, targeted section of a meeting, special invitations to general meeting, the bylaws, and other inclusive manners).   Please explain how you have done this and how it has worked for your Chapter. 

 
Chapter has welcomed Practice Administrators into their membership (i.e. special meetings, targeted section of a meeting, special invitations to general meeting, the bylaws, and other inclusive manners).   Please explain how you have done this and how it has worked for your Chapter. 

 

Education:
Chapter holds at least one annual meeting for its general membership or at least two meetings a year for targeted members. Targeted membership could be by region, specialty, membership type, institution or other demographics.  

 

What other education meetings, materials or mechanism do you offer your members? 

If the Chapter conducts patient education, please share your experience. 

Quality:
Name the ACC quality improve programs in which the Chapter is actively participating.   Check off boxes listing: STEMI/MLL, Patient Education, D2B, H2H, IC3, GAP, Other. 

 

The Chapter has worked with AHA on Mission Lifeline concerning STEMI issues.  Elaborate on accomplishments. 

 

Chapter has established a partnership with the State Medical Society.
 

Describe any projects you have worked on with the State Medical Society.  


Chapter has held a summit, conference or meeting dedicated to quality issues.

 

Chapter has held at least one meeting or meeting section dedicated to educating hospital administrators about the NCDRTM CathPCI, ACTION, ICD or CARE Registries, or educate practice administrators about the NCDRTM IC3 Registry.
 

Advocacy:
Chapter President or other Chapter leader on behalf of ACC has hosted or attended any political event within the last year and reported the event to ACC National Advocacy staff.  


Chapter President or other Chapter leader has maintained contact with legislator(s) for future ACC advocacy efforts. 

 

Chapter has organized a state lobbying effort with the help of ACC National.
 

Chapter has organized a “Cardiologist for the Day” event in their state.  

The Chapter has engaged in regulatory or legislative issues.  Please elaborate.  

 

AWARD SUMMARY
